You can request a replacement High Risk Work licence via the online portal because it has been:

• Lost
• Stolen
• Defaced
• Destroyed
• Changed your name

You will require your licence number to access the portal.

If you have changed your name, you will need to do this with SafeWork SA prior to requesting your replacement. Email your request to swsahighriskwork@sa.gov.au
